❗Required: Use MM/DD/YYYY format (ex: 12/12/2020). This will show on your certificate.

❗Required: 🧾 Got paperwork? Pop in the number from the top (it’s usually labeled Case No or Citation No). No paperwork yet or taking this for Work or Personal reasons? Just type “PRIVATE” or “EMPLOYER” or email us for any custom information needed on your certificate.
